§ 188B-9. Information to be submitted with permit application.

Each applicant must submit to the Board of Health the following information with the permit application:

A.
Proof of the tipping location for recycling and residential solid waste. These locations must be licensed processing facilities.

B.
Truck routes and delivery days. All changes must be reported to the Board of Health in writing within two weeks of the change.

C.
A copy of the registration and insurance certificates for each truck working in the Town of Blackstone.

D.
Certificates from an insurance company licensed to do business in the state of Massachusetts showing the applicant carries insurance for personal and property liability.
